Basic Settings for a School Form

Name - Remember that the name of the form is visible to recipients.

Status

• Active - Recipients can use the form.

• Inactive - Recipient can't view or enter data.

Does this form have a fee? - To enable the payment section, select Yes.

Publish date range - This determines when the form is visible to recipients.

Due date - This appears on notifications to the recipient.

Is this form student centric? - To enable other faculty to view the submitted forms, select Yes. Then choose the faculty who should have Access to view and print the responses.

For example, enable teachers to print submitted permission slips for students in their class rosters.

Note: For security and privacy reasons, you can't grant access to "medical" school forms. Only the school nurse can view and print responses for submitted "medical" school forms.

Allow user to proceed to the next form? - To streamline submitting school forms, select Yes. Whenever a user has multiple outstanding forms to submit, this enables the user to proceed directly from one complete form to the next form the user needs to complete. If you do not enable this option, users must select to open each outstanding form individually. This option is especially helpful for schools that send multiple forms to families and for families with multiple students.

Recipients - Select whether the form should go to users (such as students), parents of users, or both.

Note: If a school form is sent to multiple recipients in a family, includes a payment section but the fee is paid or waived, and all required signatures are submitted, then the form is marked as submitted.

Regarding - Select which users (or whose parents) should receive the form. To do so , select + Add and search for a user. To limit your search results, use the Role, Grade, School Year, and Group filters. Then select a user and select Add currently selected.

Status

• Active - Recipients receive a reminder by email regarding incomplete forms.

• Inactive - Recipient don't receive a reminder by email.

Reply email - When recipients receive a reminder and want to reply to the email notification, the reply goes to this address.

From name - Enter the name of the school, department, or employee who recipients should contact.

Subject - This appears in the email subject line.

Reminder option - Determine when the reminder should be sent for incomplete forms.

Email body - Enter the text content of the reminder email. Use placeholders for names which vary based on the recipient or form.

Reply email - When employees receive a notification and want to reply to it, the reply goes to this address. The reply does not go to the person who submitted the form.

Recipients - Select who should receive the notification. For each recipient, select the manager role or any role, enter a name, and press Enter.
